St. Catherine of Sienna Parish maintains official records of the sacraments celebrated within our parish, including Baptism, First Holy Communion, Confirmation, and Marriage. If you need a sacramental certificate for enrollment in a Catholic school, sacramental preparation, marriage preparation, or personal records, we are happy to assist you.

To request a sacramental record, please contact the Parish Office at (516) 352-0146.  To help us locate the record, we will need as much information as possible, this includes the individual's full name, date of birth, parents' names (for Baptismal records), and the approximate date the sacrament was received.

Please note that sacramental records are confidential and may only be released to the individual named on the record, a parent or legal guardian of a minor child, or as permitted by Church law.
